Dear Patient, Family and Friends:

Thank you for your interest in Sudbury Pines Extended Care Facility. We are an 92-bed Rehab facility, conveniently located on Route 20 in Sudbury (between Boston and Worcester). Listed below is information you may find helpful while you or your loved one is recuperating here:
 
  • Our Long Term Care (LTC)/Short Term Care (STC) Unit (Station I) can take admissions 24 hours a day, 7 days a week. Our Alzheimer's/Dementia Unit (Station II) admits patients Monday through Friday, 9am-2pm.
  • For Station I, please ask for ext. 233, for Station II; please ask for ext. 239.

  • 24 hour a day visitations.
     
  • Free parking (short distance to entrance).
     
  • Physical, Occupational, Speech, and Respiratory Therapy and activities programs are running throughout the day. You may want to call ahead to assure that your family member or friend is available for visitation.
     
  • Family members are encouraged and invited to observe therapy sessions - please call the Rehab Department for information about scheduled therapy times.
     
  • For your convenience the activity schedule is posted on a large calendar located on each station.
     
  • Sudbury Pines Extended Care prepares and serves restaurant quality food in an elegant dining atmosphere. Room service is available upon request.
     
  • Visitors are welcome to dine with the patient free of, as long as we have at least 1 hours notice.
     
  • For your convenience vending machines are located in our small dinning area, and are accessible 24 hours a day.
     
  • Children are welcome.
     
  • Pets are also welcome - however pets must be leashed. We ask that you check with the nursing staff before bringing a pet into patients' room, as roommates may have allergies.
     
  • Hairdresser/Barber services are also available on site.
     
  • Payphones can be found on each station. On Station I the payphone is located outside the small dining room and on Station II the payphone is located by the entrance to the courtyard. Individual patient telephones are available for a minimal fee.
     
  • Nurse Managers are available to patients and families to answer questions or address concerns about patient care.

    If you have any further questions, please feel free to call (800) 33-REHAB and ask for Admissions at extensions 244 or 212 or Social Services at extensions 246 or 213, they are available Monday through Friday. On the weekend call (800) 33-REHAB and ask to speak with a charge nurse.
